Group Order Form (PDF) — Use this order form if you are ordering for a homeschool group or if you are testing children from multiple families. Print and fill out this order form and mail it to the address listed on the top of the order form. If you are just ordering for your own children, order online for quicker service. All group orders must be received in writing. The orders can be submitted by mail, e-mail or fax. Please ensure you submit the orders at least 2 — 4 weeks prior to your desired testing date. Please note during the peak testing months, there is the potential for possible delays, please contact our office for more information. 2018 Group Discount - For Groups testing 10+ students there is a $3.00 discount per test regardless of the test ordered.
